The open method of graft dissection

Experienced. Expert. Ethical.

We don't cut if we don't see

Each hair follicle is very precious and invaluable for a bald person, so dissection should not damage even a single live hair follicle. In the open method, only what is clearly magnified and visible is cut.

Why it matters

Blind dissection or the open method

Older, blind methods

Generally, the older versions involve blind and quick methods of dissection, which result in the transection of follicles in large numbers.

The open method

Clearly magnified and visible things only are cut. Even though it takes more time and precision, it saves the follicles to the maximum extent possible, so the wastage of follicles is minimised.

Only a handful of surgeons practise this method.

Step by step

How the open method works at Dr. Madhu's

  1. Holding the strip open

In FUT, the donor strip is held open with four hooks while it is dissected, so every follicle stays in view. Using four hooks is one example of the open method.
  2. Slivering under the Mantis microscope

The strip is slivered lengthwise under the Mantis microscope, under immersion, on a silica gel surface.
  3. Cutting only what can be seen

Only what is clearly magnified and visible is cut, so the blade passes between follicles and not through them.
  4. Refining every graft

Each graft is then refined under Zeiss stereo microscopes, the URFUG method, so it is small enough for dense packing.
  5. Sorting and counting

The refined grafts are sorted into trays of 20 and counted, so you know exactly how many grafts you receive. See how we count grafts.

With FUE, there is no strip to dissect: grafts are taken one by one under 8× Zeiss magnification loupes, and then examined and refined under the same microscopes.

What you gain

More time and precision, more grafts saved

Fewer transected follicles

Cutting only under magnification keeps transection, the cutting through of follicles, to a minimum.

More grafts from the same donor area

When fewer follicles are wasted, more of your donor hair becomes usable grafts, which matters in large sessions.

Grafts fit for dense packing

Intact, refined grafts can be placed close together for density and a natural look.

Less blood loss

Dr. Madhu's own dissection method was developed for minimal transection and minimal blood loss.

Dr. Madhu's method. Dr. Pathuri Madhu was the first hair transplant surgeon in India to practise the four-hook method and open-method dissection. He later developed his own method of dissection, with minimal transection and blood loss, and trained the team at the centre in it.

Questions

Open method: frequently asked questions

What is the open method of graft dissection?

It is a way of dissecting the donor strip in which only what can be clearly seen under magnification is cut: we don't cut if we don't see. It aims not to damage even a single live hair follicle.

How is it different from older methods?

Older methods used blind, quick dissection, which transected (cut through) follicles in large numbers. The open method takes more time and precision, but saves as many follicles as possible.

Is the open method used for FUE too?

The open method is how a FUT donor strip is dissected. With FUE, grafts are taken one by one under 8× Zeiss magnification loupes, and every graft, from FUE or FUT, is then examined and refined under the microscopes.

Who developed the method used at Dr. Madhu's?

Dr. Pathuri Madhu first practised the four-hook method and later developed his own dissection method, with minimal transection and blood loss. The team at the centre is trained in it.
