Older, blind methods
Generally, the older versions involve blind and quick methods of dissection, which result in the transection of follicles in large numbers.

We don't cut if we don't see
Each hair follicle is very precious and invaluable for a bald person, so dissection should not damage even a single live hair follicle. In the open method, only what is clearly magnified and visible is cut.
Why it matters
Generally, the older versions involve blind and quick methods of dissection, which result in the transection of follicles in large numbers.
Clearly magnified and visible things only are cut. Even though it takes more time and precision, it saves the follicles to the maximum extent possible, so the wastage of follicles is minimised.
Only a handful of surgeons practise this method.
Step by step
With FUE, there is no strip to dissect: grafts are taken one by one under 8× Zeiss magnification loupes, and then examined and refined under the same microscopes.
What you gain
Cutting only under magnification keeps transection, the cutting through of follicles, to a minimum.
When fewer follicles are wasted, more of your donor hair becomes usable grafts, which matters in large sessions.
Intact, refined grafts can be placed close together for density and a natural look.
Dr. Madhu's own dissection method was developed for minimal transection and minimal blood loss.
Dr. Madhu's method. Dr. Pathuri Madhu was the first hair transplant surgeon in India to practise the four-hook method and open-method dissection. He later developed his own method of dissection, with minimal transection and blood loss, and trained the team at the centre in it.
Questions
It is a way of dissecting the donor strip in which only what can be clearly seen under magnification is cut: we don't cut if we don't see. It aims not to damage even a single live hair follicle.
Older methods used blind, quick dissection, which transected (cut through) follicles in large numbers. The open method takes more time and precision, but saves as many follicles as possible.
The open method is how a FUT donor strip is dissected. With FUE, grafts are taken one by one under 8× Zeiss magnification loupes, and every graft, from FUE or FUT, is then examined and refined under the microscopes.
Dr. Pathuri Madhu first practised the four-hook method and later developed his own dissection method, with minimal transection and blood loss. The team at the centre is trained in it.
