Clear, Healthy Scalps at Dr. Madhu's Advanced Hair Transplant Centre
Dandruff is one of the most common scalp conditions, often beginning in the teenage years and extending throughout adulthood. While it is predominantly restricted to the scalp, it can occasionally affect the beard, mustache, chest hair, and shoulders.
Maintaining a healthy scalp is a critical foundation for robust hair growth and successful hair restoration procedures.
In Hyderabad, we see dandruff flare up for many patients during the hot summer and the humid monsoon months, when sweat and oil build up on the scalp. If you have tried several anti-dandruff shampoos and the flakes keep coming back, the problem may not be ordinary dandruff at all, and that is where a proper dandruff treatment plan from a doctor makes the difference.
Diagnosis first
Differentiating dandruff from other scalp conditions
Not all white flakes are simple dandruff. It is clinically important to differentiate standard dandruff from other underlying dermatological conditions that may require specific medical intervention:
As a rough guide, dandruff flakes tend to be oily or yellowish and itchy, dry scalp flakes are fine and powdery, and psoriasis scales are thick, silvery and sharply bordered, sometimes extending past the hairline. If your flakes return within a day or two of washing, bleed when scratched, have spread to the eyebrows, beard or behind the ears, or have not improved after four weeks of a medicated shampoo, it is worth seeing a dermatologist instead of switching shampoos again.
Hair loss and surgery
Dandruff and hair transplantation
Many patients wonder how dandruff affects hair loss and their candidacy for a transplant. It is crucial to understand that dandruff is a separate entity from Androgenetic Alopecia (genetic pattern baldness).
Dandruff on its own does not cause permanent baldness. Constant scratching and untreated scalp inflammation can, however, lead to temporary hair fall and breakage, which usually settles once the scalp is under control.
If you are planning to undergo a hair transplant, severe dandruff or scalp inflammation must be properly evaluated by a dermatologist and brought under control beforehand. To ensure the scalp is in an optimal, healthy state for surgery, we often recommend targeted scalp care, such as the use of medicated shampoos containing active ingredients like salicylic acid and ketoconazole, or povidone-iodine.
Medicated shampoos work best when the lather is left on the scalp for about 5 minutes before rinsing, used two or three times a week or as your doctor advises. Some flaking and crusting after a transplant is a normal part of healing and is not dandruff; your post-operative care plan explains how to wash the grafted area safely.
Our approach
Dandruff treatment at our Jubilee Hills centre
Every dandruff consultation at our centre starts with a close look at the scalp to find out what is actually causing the flakes. From there, the plan may include:
- A clinical scalp examination to tell dandruff apart from seborrheic dermatitis, psoriasis, fungal infection or dry scalp
- A prescription shampoo and lotion routine matched to your scalp type
- Treatment for related areas such as the beard, eyebrows or chest when needed
- A simple maintenance routine so the dandruff stays under control
- Pre-surgery scalp preparation for patients planning a hair transplant

Questions
Frequently asked questions about dandruff
What causes dandruff?
Dandruff is mainly caused by an overgrowth of Malassezia, a yeast that lives on most people's scalps and feeds on scalp oil. Oily skin, sweat, stress, infrequent hair washing and cold, dry weather can all make it worse. It is not caused by poor hygiene alone.
Can dandruff cause hair loss?
Dandruff does not cause permanent hair loss or baldness. Heavy itching, scratching and scalp inflammation can cause temporary hair fall, which usually improves once the dandruff is treated. If you are losing hair steadily from the crown or temples, that is more likely pattern baldness and should be checked separately.
How do I get rid of dandruff permanently?
Most dandruff cannot be cured permanently, but it can be controlled very well. A medicated shampoo with ketoconazole, salicylic acid, zinc pyrithione or selenium sulfide clears most cases in a few weeks, and using it once a week afterwards helps prevent it coming back. Stubborn cases need a doctor's prescription.
Which shampoo is best for dandruff?
The best dandruff shampoo depends on the active ingredient, not the brand. Ketoconazole works against the yeast behind dandruff, salicylic acid loosens thick flakes, and zinc pyrithione and selenium sulfide also slow the fungus. For dandruff that keeps returning, a dermatologist can prescribe a stronger formulation suited to your scalp.
Is dandruff contagious?
No, dandruff is not contagious. The yeast linked to dandruff is present on most people's skin, so you cannot catch dandruff by sharing a comb, pillow or towel. Flaking that spreads between family members is more likely to be head lice or a fungal infection and should be checked.
Why do I get dandruff in my beard and eyebrows?
Dandruff can appear wherever there are oil glands and hair, including the beard, mustache, eyebrows, chest and behind the ears. Flaking in these areas is often seborrheic dermatitis. Washing the beard with the same medicated shampoo used on the scalp usually helps, but the skin around the eyes needs gentler, doctor-advised care.
